<p data-start="125" data-end="293">For a brief moment, day and night stand in perfect balance. Light and darkness share the sky equally, as if the earth itself has paused in the movement between seasons.</p>
<p data-start="295" data-end="326">Nothing is rushing forward yet.</p>
<p data-start="328" data-end="541">In the Northern hemisphere winter has not completely released its hold, and spring has not fully taken the lead. The land holds this moment of equilibrium — a still point in the cycle — as though everything is gathering itself in readiness.</p>
<p data-start="543" data-end="597">Then, almost imperceptibly, the balance begins to tip.</p>
<p data-start="599" data-end="668">The light begins to take the lead, and the earth moves toward growth.</p>
<p data-start="670" data-end="735">Across the land, fertility begins to awaken from winter dormancy.</p>
<p data-start="737" data-end="1003">Buds swell along branches that appeared lifeless only weeks ago. The soil softens and warms. <p data-start="1431" data-end="1537">In pagan traditions, this moment of balance is honoured as <strong data-start="1490" data-end="1500">Ostara</strong>, the festival of the Spring Equinox.</p>
<p data-start="1539" data-end="1861">Ostara marks the point where the wheel of the year crosses a threshold. Winter’s stillness gives way to the momentum of the growing season. The symbols associated with the festival — the egg and the hare — reflect ancient recognition of fertility, reproduction, and the generative force of life awakening across the earth.</p>
<p data-start="1863" data-end="1919">Yet the deeper meaning of the Equinox is balance itself.</p>
<p data-start="1921" data-end="2082">Before growth accelerates, the earth pauses here. Light and darkness share the sky equally. The system recalibrates before the energy of spring gathers momentum.</p>
<p data-start="2084" data-end="2124">Human physiology recognises this rhythm.</p>

				<div class="et_pb_text_inner"><p>Publicly funded assisted human reproduction (AHR) has been live in Ireland since 2023. Fertility treatment that was once entirely private is now partially funded through the HSE and delivered through the public fertility pathway.</p>
<p>During this same period, fertility legislation has been enacted and the Assisted Human Reproduction Regulatory Authority (AHRRA) has been formally established.</p>
<p>However, while funding is live and treatment is being delivered, system-level outcome regulation has not yet been embedded. In particular, live birth is not measured as a tracked outcome across the publicly funded fertility pathway.</p>

<h2>How publicly funded fertility care currently operates</h2>
<p>The State allocates approximately €30 million annually to Ireland’s public fertility Model of Care, as defined by the HSE and the Department of Health.</p>
<p>This funding supports a multi-stage pathway. Patients enter through primary care, progress to HSE fertility hubs for assessment and management, and for a subset of patients, are referred onward for Assisted Human Reproduction (AHR).</p>
<p>AHR treatment — including IUI, IVF, and ICSI — is publicly funded but delivered by private fertility clinics under HSE funding arrangements.</p>
<p>According to the HSE’s most recently published figures, approximately one third of patients referred through the public fertility pathway progress to AHR.</p>
<p>The remaining majority complete the pathway without achieving a live birth. HSE-published data indicates that approximately 70% of patients entering the publicly funded fertility pathway do not achieve a live birth, regardless of whether they progress to AHR</p>

<h2>Regulation: established in law, limited in operation</h2>
<p>The Health (Assisted Human Reproduction) Act 2024 provides the statutory framework for fertility regulation in Ireland. Under this Act, the Assisted Human Reproduction Regulatory Authority (AHRRA) has been formally established.</p>
<p>The Authority is intended to license fertility clinics, set enforceable standards of practice, oversee compliance, and regulate donor-assisted conception and surrogacy.</p>
<p>However, while the Authority exists in statute, system-level regulatory functions are not yet fully operational across publicly funded fertility care. In particular:</p>
<ul>
<li>fertility clinics delivering publicly funded AHR are not yet operating under a mature, inspection-led regulatory regime</li>
<li>there is no established system of routine, regulator-verified clinic inspections specific to fertility care</li>
<li>there is no mandatory, system-wide public reporting of outcomes across the publicly funded fertility pathway</li>
</ul>
<p>Publicly funded fertility treatment is therefore being delivered through outsourced private providers without an established framework for system-level accountability.</p>

<h2>Live birth: the outcome patients are seeking</h2>
<p>Patients enter the HSE fertility pathway seeking a live birth. That outcome — not access, referral, or treatment initiation — is the measure that matters to those using the system.</p>
<p>At present, live birth is not tracked as a system-level outcome across the publicly funded fertility pathway, including outsourced AHR provision.</p>
<p>This means that neither patients nor the State can evaluate whether publicly funded fertility care is delivering the outcome it is designed to achieve.</p>
<p>Without live birth as a tracked system outcome:</p>
<ul>
<li>patients cannot assess the likelihood of success when entering the pathway</li>
<li>performance cannot be compared across providers or stages of care</li>
<li>improvement cannot be measured over time</li>
<li>accountability remains diffuse</li>
</ul>
<p>This is not a judgement on individual clinicians or clinics. It is a structural observation about what the system measures — and what it does not.</p>

<h2>Why outcome measurement matters in a funded system</h2>
<p>Public funding creates obligations beyond eligibility criteria. It requires the ability to assess whether the system is functioning effectively.</p>
<p>In the absence of outcome measurement, patients are asked to enter the publicly funded fertility pathway without visibility on the likelihood of achieving a live birth.</p>
<p>At a policy level, decisions about access and funding are therefore made without reference to verified outcome data.</p>

<h2>How the UK system differs</h2>
<p>The UK offers a useful comparison.</p>
<p>Fertility clinics in the UK are regulated by the Human Fertilisation and Embryology Authority (HFEA), an independent statutory regulator with operational oversight.</p>
<p>The HFEA licenses clinics, conducts inspections, and publishes verified clinic-level data, including treatment volumes, pregnancy rates, live birth outcomes, and inspection findings.</p>
<p>This data is publicly accessible and updated on a routine basis.</p>
<p>The difference between the Irish and UK systems is not funding alone. It is the presence of operational regulation, outcome transparency, and system-wide visibility. </p>

<h2>Final reflection</h2>
<p>Publicly funded fertility care represents an important shift in Irish healthcare.</p>
<p>That does not make scrutiny premature. It makes it necessary.</p>
<p><strong>Live birth is the outcome patients enter the HSE fertility pathway seeking.<br />It must also be the outcome the system is designed to measure.</strong></p>
<p>In the absence of system-level live birth measurement,  evidence-based preparation becomes the primary mechanism through which outcomes can still be positively influenced. Preparation is not about adapting to system constraints. It is about shaping the biological conditions that determine whether fertility treatment has the capacity to succeed — including metabolic health, inflammatory load, nutrient sufficiency, sperm quality, and cycle function — <em data-start="789" data-end="797">before</em> eligibility thresholds, waiting times, and treatment decisions narrow what is possible. In a system that emphasises assessment and triage rather than preparation for success, this phase becomes the primary opportunity for influencing outcome.</p></div>

<p>There is a particular kind of waiting inside the public fertility pathway — one that doesn’t feel passive or neutral. When you’re trying to conceive, time isn’t counted in months but in cycles, and each one carries its own emotional and physical load. So when appointments sit weeks or months apart, it doesn’t feel like ordinary waiting. It feels like life being held in suspension.</p>
<p>This weight doesn’t fall on one partner alone. Fertility is a shared physiology, and both partners are affected by delays, even though public appointments tend to focus more heavily on the woman’s cycle.</p>
<p>Across Ireland, for most couples, HSE fertility wait times range from eight to twenty-four weeks for their first <a href="https://www.hse.ie/eng/about/who/acute-hospitals-division/woman-infants/national-fertility-services/fertility-services-report-2023-2024.pdf">Fertility Hub</a> appointment. Imaging such as HyCoSy or HSG can add another six to twelve weeks. Blood tests may be completed within two to six weeks, but the follow-up appointment that interprets them often sits another eight to sixteen weeks away. If ovulation induction or additional investigation is recommended, this may involve another four-to-twelve-week pause. And if the next step is referral for publicly funded Assisted Human Reproduction (AHR), the first clinic appointment typically adds another twelve to twenty weeks.</p>
<p>These pauses don’t reflect anything about you. They reflect the system’s capacity. But the emotional strain lands on the people who are waiting — particularly those facing <a href="https://nowbaby.ie/secondary-infertility/">secondary infertility</a> or trying again after <a href="https://nowbaby.ie/miscarriage/">pregnancy loss</a>.</p>
<p>As time passes, the body responds to uncertainty. Sleep may lighten. Stress hormones may remain elevated. Cycles can feel less predictable — not because something is wrong, but because prolonged uncertainty has physiological effects. Male fertility is also responsive to time and load; sperm quality is dynamic, not fixed. Couples previously told they had <a href="https://nowbaby.ie/unexplained-infertility/">unexplained infertility</a> often discover that important influences were never assessed.</p>
<p><strong>This is why waiting feels so heavy. It seems as though you can only tread water, while the clock continues to tick.</strong></p>

<p data-start="532" data-end="572">Waiting alone does not optimize success.</p>
<p data-start="574" data-end="725">In the HSE fertility pathway, only around 30% of couples ultimately take home a baby. When outcomes are uncertain, and there is only one funded cycle, deliberate preparation is your best chance to improve your outcome.</p>

<h2>The Weight of the Three Miscarriage Rule</h2>
<p>Most women don’t discover the three-miscarriage rule until they are already grieving. It sits quietly in the background of fertility care in Ireland and the UK, rarely spoken about openly, rarely explained, rarely acknowledged for what it is: a threshold that asks women to endure repeated loss before anyone looks beneath the surface.</p>
<p>And long before three losses, the emotional cost has already begun.</p>
<p>For many, the first <a href="https://nowbaby.ie/miscarriage/">miscarriage</a> is a collision between hope and a reality they never saw coming. They were never told how common early loss is. They were never told that most pregnancies end before anyone even knows they existed. They were never told that miscarriage is woven into the fabric of human reproduction. And so when it happens, it feels like a rupture — a break in the future they were already beginning to imagine.</p>
<p>I understand personally that pregnancy is not always a straight line — <strong>I have one baby, but four pregnancies.</strong></p>
<p>And still, like so many women, I carried the quiet fear that loss meant something was wrong with <em>me</em>.</p>
<p>There is a pervasive belief that miscarriage is a woman’s failure: her body didn’t hold the pregnancy, her hormones weren’t strong enough, she couldn’t do what other women seem to do so easily. But early loss is rarely about a woman’s ability to carry. In truth, miscarriage is just as likely to arise from sperm as from egg, and often from factors neither partner could have known or influenced. It is not a test of strength or capability. It is not a verdict. It is not a reflection of your worth. And it is never — ever — a woman’s fault.</p>
<p>The first loss is not “one.” It is a shattering of innocence.</p>
<p>It is the moment hope becomes cautious.<br />The moment joy becomes something you hold lightly.<br />The moment your relationship with pregnancy changes forever.</p>
<p>Women begin counting long before the system does.<br />“Please don’t let this be the start of three.”<br />A sentence whispered internally, silently, fearfully — even after just one loss.</p>
<p>And this is where the three-miscarriage rule leaves its first quiet bruise.</p>
<p>Because this rule does not simply determine access to care. It shapes the emotional landscape of the months and years between pregnancies. It conditions women to brace for repeated heartbreak. It teaches them to monitor their loss as a number rather than a grief.</p>
<p>It tells women that the first miscarriage is not enough.<br />The second is not enough.<br />Only the third unlocks the right to ask, “Why?”</p>
<p>But grief does not become more valid the third time it happens. It is valid the first time. It is valid every time.</p>
<p>And in the space between pregnancies, women carry far more than anyone realises.</p>
<p>They carry hypervigilance.<br />They carry the instinct to not get too attached.<br />They carry the dread of symptoms disappearing — or appearing.<br />They carry the fear of telling anyone.<br />They carry the terror of telling themselves.<br />They carry the internal countdown: one… two… please don’t let there be three.</p>
<p>This is the emotional cost of a rule that was never designed with women’s hearts in mind.</p>
<p>If you are reading this after a loss, it may be helpful to know that early miscarriage is heartbreakingly common, and often linked to chromosomal factors that neither partner could have changed. You are not alone in this, even if it feels that way. You can find general medical information on miscarriage from trusted sources such as the <a href="https://www.nhs.uk/conditions/miscarriage/" target="_blank" rel="noopener">NHS miscarriage guidance</a>, but information is only one part of what’s needed. The other part is having your experience acknowledged as real, valid and worthy of care — from the very first loss.</p>
<p>But here is the part that matters most: <strong>you do not need to wait for the system to catch up to your grief.</strong></p>
<p>The months between pregnancies are not empty time. They are living space — the place where eggs are maturing, hormones are rebalancing, inflammation is shifting, and resilience is being rebuilt. Three months of focused preparation can meaningfully support the internal environment a pregnancy grows from. It doesn’t erase the past, and it cannot guarantee the future — but it strengthens the ground beneath you.</p>
<p>You do not have to earn the right to support by losing something you love three times.</p>

<p>The HSE fertility hub is where most couples begin their journey through Ireland’s publicly funded fertility system.</p>
<p>You’ve already done what so many never get the chance to do.<br />You cleared the HSE’s maze of <a href="https://nowbaby.ie/hse-fertility-hub-age-limits/">age limits</a>,<a href="https://nowbaby.ie/hse-fertility-hub-bmi-restrictions/"> BMI targets</a>, and referral waiting lists. You survived the blood tests, the consultations, the waiting rooms. You made it all the way to treatment — maybe to egg collection, maybe to embryos, maybe even to transfer.</p>
<p>That’s not small.<br />That’s persistence in a system designed to filter people out.</p>
<p>The HSE fertility hub pathway is the route couples navigate before reaching this point, but what happens after that referral is rarely explained. The cycle begins, and it reaches its outcome — whether that’s no embryos, an unsuccessful transfer, a clinical pregnancy that doesn’t continue, or a birth. And when that outcome arrives, your position in the system is determined entirely by one thing: whether any embryos remain. If none exist, your funded care ends immediately. If embryos do exist, you stay within the pathway only until those embryos lead to a live birth — or until they run out.</p>
<p>It is only when you see these quiet rules in action that the realisation lands: the system gives you one full cycle to try for a baby — and that cycle has a 70 percent chance of ending without one.</p>
<h2>The 70 Percent Reality</h2>
<p>When Ireland’s publicly funded IVF pathway launched, it was celebrated as progress — finally, a chance for equality in access.<br />But hidden beneath the headlines is the harder truth:</p>
<p><strong>One funded round cannot overcome the biology of reproduction, the demands of IVF, or the absence of preparation support.</strong></p>
<p>And that is where the 70 percent reality becomes unavoidable.</p>
<p>Between September 2023 and August 2024, 1,084 couples were referred for publicly funded assisted treatment (IUI, IVF or ICSI). By the time outcomes were reported, only 206 had completed their treatment — and just 79 had a confirmed clinical pregnancy at the 6–8-week scan.<br />That’s an early pregnancy rate of 38 percent.</p>
<p>But early pregnancy is not a baby.<br />When you apply the normal drop-off between early pregnancy and live birth — seen across every IVF dataset worldwide — the real-world live-birth outcome from one funded cycle is likely to settle closer to <strong>25 percent</strong>.</p>
<p>Which means:</p>
<ul>
<li><strong>around 25 percent will take home a baby</strong></li>
<li><strong>around 75 percent will not</strong></li>
</ul>
<p>Rounded, this is the <em>70 percent failure rate</em> — the number that defines the entire scheme.</p>
<p>Not because couples failed.<br />Not because their embryos failed.<br />But because the structure of a one-cycle system creates a statistical bottleneck few can overcome.</p>

<h2>What the HSE Fertility Hub Doesn’t Explain</h2>
<p>The public scheme sounds straightforward:</p>
<p>“One fresh cycle and as many frozen cycles as needed to have a baby.”</p>
<p>In practice, it is far more limited.</p>
<h3>If no embryos are created</h3>
<ul>
<li>There are no frozen cycles</li>
<li>You are immediately discharged</li>
<li>There is no funded second attempt</li>
</ul>
<h3>If embryos are created but do not implant</h3>
<ul>
<li>Frozen transfers are funded</li>
<li>Only until:
<ul>
<li>a live birth occurs, or</li>
<li>the embryos run out</li>
</ul>
</li>
</ul>
<h3>If a clinical pregnancy occurs but later fails</h3>
<p>You remain in the funded pathway <strong>only if frozen embryos remain</strong>.<br />Frozen transfers continue until:</p>
